Conclusion helps Prescan power preventive health insights

By unifying data into a single source of truth on Microsoft Fabric, Prescan can operate more efficiently and, ultimately, improve patient outcomes.

October 27, 2025

Today, preventive healthcare plays an increasingly important role in helping people live longer, healthier lives. But as demand grows, clinics and providers face a difficult challenge: how to scale and coordinate their services while tracking complex data like patient and financial information, scheduling, and clinical outcomes. These are often recorded in different systems, and stitching them together into a single source of truth is no small feat.

All too often, healthcare providers are held back by limited access to reliable, real-time insights. In sectors where precision and trust are everything, organizations need a partner who understands the nuances of regulated, data-heavy environments.

With more than 30 years of experience developing business-critical IT solutions, Conclusion specializes in building bespoke, data-driven applications for sectors where reliability, auditability, and security are nonnegotiable. As a Microsoft partner for more than a decade, Conclusion’s deep expertise in cloud migration, semantic modeling, and data intelligence makes them a trusted advisor for organizations across Europe in healthcare, logistics, government, and beyond.

When Prescan, a market leader in preventive medical diagnostics in the Netherlands, wanted to build a future-proof data platform, Conclusion was the logical partner to help make it happen.

Prescan’s growing demand for intelligent insights

Since 2003, Prescan has pioneered preventive health diagnostics, giving individuals and corporate clients early insights into their health. With multiple clinics and offices, the company guides patients through every step of the journey, from orientation and preparation to the day of examination and into aftercare. Their mission is to empower people with early insights into their health through high-quality, accessible medical examinations.

While their mission is straightforward, their operations are complex. Prescan used a number of systems to manage business operations and stages of the patient journey. These systems operated largely in silos, making it difficult to combine information into a clear, unified picture of business performance and patient journeys.

Prescan saw an opportunity to take their operations to the next level. By connecting insights into performance, capacity, and patient flows, they could move beyond siloed views and gain holistic insights to guide strategy, accelerate decision-making, and simplify planning.

Recognizing the need for a unified, secure, intelligent data environment—and knowing that they needed it quickly—Prescan turned to Conclusion for their engineering expertise and experience delivering critical Azure-based data solutions.

A future-ready data platform with Microsoft Fabric

Working together, Conclusion and Prescan set out to design a platform that could serve as the backbone of Prescan’s data-driven future. Built on Microsoft Fabric, the new platform integrates ingestion, storage, processing, and visualization into a single SaaS-based analytics environment that’s ideal for Prescan’s compact IT team and cloud-first approach.

The platform is powered by three key, distinct elements:

  • Semantic data modeling. Conclusion designed a semantic model that unified data definitions across disparate systems. This allowed Prescan to combine operational and financial data into consistent, scalable analytics without disrupting existing workflows.

  • End-to-end data architecture. The team implemented a medallion architecture with a data lakehouse, enabling step-by-step data enrichment. This gave Prescan reliable insights into planning, performance, and capacity, forming the basis for KPIs that improve care quality.

  • Power BI integration. The platform supports reliable and scalable Power BI dashboards, which Prescan management can use to monitor KPIs and performance metrics across departments.

The collaborative process proved just as important as the technology itself. The agile working relationship between Prescan and Conclusion allowed them to align technical implementation with business priorities at every stage, ensuring that the solution not only solved today’s problems but also positioned Prescan for the next wave of innovation.

“We now have better control over our processes and can steer based on data,” said Edwin Kortleve, Manager, Commercial & Digital at Prescan. “Conclusion helped us build a future-proof platform that grows with our ambitions.”

Enabling data-driven healthcare and future innovation

With the new architecture in place, Prescan’s leadership team can access real-time insights across the entire patient journey, enabling their teams to make even more informed decisions and allocate resources more efficiently. Among the immediate benefits were:

  • Improved operational steering with reliable, real-time insights into patient flow and capacity.

  • Faster decision-making thanks to semantic modeling and metadata-driven pipelines.

  • Scalable data architecture that makes it easy to onboard new sources without disruption.

  • Reliable Power BI dashboards used across departments to monitor KPIs and performance.

“The collaboration between Prescan and Conclusion led to tangible improvements in data accessibility, operational control, and strategic decision-making,” said Robbrecht van Amerongen, Strategy Director at Conclusion. “These outcomes were achieved through the implementation of a Microsoft Fabric-based data platform, which unified data from multiple systems into a single, intelligent architecture.”

Looking ahead, Prescan plans to build on this foundation to further enhance service quality and efficiency. With Microsoft Fabric as the backbone and Conclusion as their trusted partner, they are poised to expand the platform with new sources, advanced analytics, and AI-driven insights. As the demand for preventive diagnostics continues to grow, Prescan’s ability to scale intelligently will be critical both for the success of their operations and to achieve the best patient outcomes. Now they have ready access to the data they need to do it.
